2. To submit, click on the 'Submit by Email' button above and below this document. You will not be allowed to save a copy for your records so we recommend you click on the 'Print Form' button for your records.

Contact/Presenter Information Sheet & Workshop Proposal must be submitted by April 2, 2012. All submissions will receive an electronic notification of receipt. All accepted proposals will receive individual notification no later than June 1, 2012.

Note: Since the conference is provided by a non-profit organization all presenters are required to cover their own conference costs including any transportation, hotel accommodations and meals. However, conference registration fees will be complimentary for each primary presenter and for a limited number of panel participants and co-presenters upon request. Networking opportunities for presenters will be provided.
